When Is the Best Time for Plastic Surgery?

You want to make sure you are fully healed before a big event, such as a wedding, and that you do not have any major commitments planned during your recovery time. Here are a few things to consider before choosing your ideal surgery date.

Age

For one reason or another, many people worry that they are too old for plastic surgery. Patients should know that age alone will never disqualify anyone from a particular procedure; however, older patients are more likely than others to have more serious health concerns, which may make elective surgery more of a risk. If you are considering plastic surgery and already feel “too old,” schedule a consultation to find out the pros and cons that are unique to you.

Future Weight Loss

Those who experience the best results with all body contouring procedures are at or near their ideal weight and have maintained it for at least a few months. Patients who plan on losing more weight after their surgery will still be able to schedule their surgery, but once they lose more weight, their body may not be as “tight” as their final surgery result, as excess skin may develop.

Future Pregnancy

Cosmetic surgeries such as the tummy tuck will never cause harm to the woman or baby if the patient becomes pregnant after her surgery. Nonsurgical procedures are an excellent option for last-minute improvement before a big event. The Pros and Cons of Breast Augmentation With Fat Transfer

lady-measuring-her-breastsIn all honesty, there are advantages and disadvantages to virtually all cosmetic procedures. However, the true list of pros and cons is completely determined by the patient and will be different for every individual; an ideal procedure for one person may not be the best option for another. Here are the facts about breast augmentation with fat transfer to help you determine your particular list of pros and cons:

It Reduces Body Fat

Perhaps the best and most obvious advantage of breast augmentation with fat transfer is the loss of fat from other areas of the body. The donor area can be the thighs, hips, buttocks, or abdomen—almost any larger area of the body that has excess, unwanted fat. Patients experience a few inches lost from the particular area and can feel more confident in smaller or tighter clothing.

It Is Less Invasive Than Implant Placement

Breast augmentation with fat transfer is less invasive than implant breast augmentation depending on the amount of fat removed during the liposuction portion of the procedure. Liposuction will require only a few small incisions at the donor site but may result in swelling and bruising that will cause some discomfort during recovery. The surgical step of injecting the fat into the breasts is much less invasive than surgically inserting implants. Generally, the fat grafting procedure results in a shorter and easier recovery than implant augmentation.

It Has Lower Risks Than Implant Augmentation

With breast implants, patients run the risk of capsular contracture, implant malposition, rippling, and rupture. With fat transfer to the breasts, the patient avoids all implant-associated risks. The body also will not reject the patient’s own fat as a result of fat transfer.

The Breasts Look and Feel More Natural

Because the patient’s own fat is used, the breasts will look and feel more natural compared to silicone or saline breast implants. For those who would like more defined roundness and cleavage at the upper pole of the breasts, breast implants may be a better option.

When fat is injected into the body, it must form a blood supply to survive. Only about 50 to 70 percent of the injected fat is likely to survive in the breasts, which can lead to less dramatic results than with breast implants. After four to six weeks, the size of the breasts stabilizes. As long as you maintain your weight, the breast size will remain the same. If you gain weight, the breasts will increase in size. They will decrease in size if you lose weight.

It Provides a Less Significant Increase in Breast Size

Patients should expect an increase of about one to two cup sizes with fat transfer, while breast implants provide many more size options. Patients can choose to get more than one fat injection treatment to achieve the size they want, but this option often takes longer than breast implant surgery and may end up costing more as well.

You look down and it although you hate to admit it, almost feel like as if you have a pair of saggy water balloons attached to your chest. A great bra may provide a temporary lift for a quick fix, but often the battle is truly apparent when you’d like to wear a little less. This descent of breast tissue is an unfortunate side effect of the natural aging process, but can also be caused by genetics, weight change, and having children.

But, you can take steps that will help your chest defy gravity. Having a Mastoplexy, or surgical breast lift, is an artful procedure to bring life back to breasts that are experiencing a number of problems created over time. Techniques Used for Breast Lifts

The three most commonly used methods of breast lift surgery are classified according to the way Dr. Agullo arranges the removal of excess skin and the incision methods associated with that technique. With each of these methods, it’s important to educate yourself about the specific plan for any breast rearrangement, and how it will impact your breast tissues.

Depending on your specific anatomical needs and preferences, you can choose between:

  • Traditional breast lift incisions
  • Vertical breast lift incisions
  • Circumareolar breast lift incisions

With the first method, traditional incisions border the outer edge of the nipple. Dr. Agullo begins with an incision near the bottom of the nipple line which extends to the lower fold of breast tissue, and an incision along the same fold. This method makes a kind of anchor shape, and may be also go by the name of “anchor lift” because of pattern that is left behind. This technique most benefits those individuals with extremely limp breasts.

For women with a moderate amount of sag, the vertical breast incision style is most appropriate. This process, like the last, brings the incision around the outside of the nipple area, then from there it extends to the fold of the lower breast tissue. It is often nicknamed the “lollipop technique” due to the pattern and shape of the incisions that are used.

And finally, breast lift incisions for the third type are often referred to as “donut” incisions. Circumareolar breast lifts are limited to the nipples, only affecting the tissues around the nipple’s outer edge. This operation is usually recommended for women with mild to moderate sagging. This process doesn’t rearrange a large amount of tissues within the breast, providing results that are a bit less intense.

Combining a breast lift with another surgery, such as breast augmentation, may be recommended to reach the best possible outcome.

Remember to consider any scarring patterns, and preferably compare post-op pictures 2 years out from the initial surgery. Misconceptions About Breast Augmentation and Lifts

shutterstock_256814590There are many reasons why some women may want to undergo augmentation mastopexy, or a breast lift – a procedure where excessive skin around the breasts is removed, to tighten and contour the overall chest area.

The results of a breast lift improve erase concerns such as stretching or drooping that commonly occur after a woman has had children, breastfed, lost weight, or aged. I typically recommend it for my patients that are experiencing sagging and reduction in the fullness of their breasts, along with procedures like breast implants. Yet, there are still a lot of misunderstandings surrounding this procedure – and many women wonder if it is something that they’ll actually need.

Common Misconceptions About Breast Lifts

There are two misconceptions that we frequently hear our patients bring up, and it is critical to clarify the information around these before any patient undergoes treatment here at Southwest Plastic Surgery. The first misconception I hear is, “I’ll only need one surgery, because I’m getting my lift and augmentation done simultaneously.”

In reality, this process involves two different processes to make the breast larger in the areas requiring fullness and smaller in the areas experiencing sagging. For some, not all patients, it is safest to have a series of procedures instead of forcing it into just one. This depends on the level of sagging, and also allows the body to heal itself on a more gradual scale.  Having a series of procedures can reduce risk of complications, such as reduced nipple sensation.

Before and After Breast LiftThe second misunderstanding regards scarring. “From what I hear, it’s just supposed to be an incision around the areola and nipple area, so scarring won’t happen.” Each person is different, which means their breasts and skin are different. Plastic surgery will always leave some type of scar; they can be minimal, or more prevalent. This is the trade off of cosmetic procedures: getting a scar, or not having the procedure done. Individuals may have their incisions in different locations than expected, but that can be discussed prior to surgery. Patients with more sagging will, generally, require more incisions to create a breast lift. Less sagging, on the other hand, will require fewer incision sites.

Reasons to Revise Your Breast Augmentation

Breast Augmentation RevisionBreast augmentation surgery is one of the most common and routine plastic surgery procedures performed in the U.S. With newer implant technologies and better techniques, more and more women are able to receive the breast enhancement they desire. However, your breast augmentation may not have yielded the results you envisioned. A breast augmentation revision is performed to provide women with the breasts they wished for from their first breast augmentation. There is a wide range of reasons why women may choose to have their breast augmentation revised.

Breasts That Are Too Large or Too Small

Breast Augmentation Before and After PhotosOne of the most common reasons women choose to revise their breast augmentation is because it didn’t result in the breast size they wanted. For some women, the implants they chose resulted in breasts that were too large. Either they were disproportionately sized for her body, or they were too heavy and cumbersome. More often, women feel that their breast augmentation didn’t achieve as much volume as they were expecting. They may have felt that their breasts were still too small and didn’t provide the curves and enhancement they envisioned.

Implants that are too large or too small can be removed, and more appropriately sized implants can be inserted in their place. Women who decrease the size of their implants may require a minor removal of excess breast tissue to help achieve better results.

Implant Rupture or Leak

While implants are made to be very durable, trauma to the implant or a natural defect can occasionally cause ruptures or leaks. Rupture of a saline implant is more immediately noticeable; the breast will appear deflated due to the harmless saline solution leaking out of the implant. Contrarily, silicone implants require an MRI to detect implant leaks.

Implant Displacement

Over time, breast implants can rotate within their created pockets. In some cases, the breast tissue becomes too weak to support the implants, and they begin to move out of the breast pocket. In these cases, revision surgery will reposition the implant. Occasionally, heavy implants can be replaced with smaller, lighter ones, and a minor breast lift can be performed.

Symmastia, or the inward lateral movement of the breast implants, contributes to the appearance of a singular breast. Correcting symmastia creates better breast definition and enhances cleavage.

Capsular Contracture

Capsular contracture occurs when the natural shell formation around the breast implant begins to contract and squeeze the implant. This can be painful and can cause the breasts to look and feel hard. A surgical procedure is used to release the hardened capsule, and in some cases, all the scar tissue will be removed.

Other Reasons

Other health risks and complications can lead to the need for a breast augmentation revision. These reasons may pose health risks that need to be addressed. However, many reasons involve improving the aesthetic appearance of the breast. No matter your personal reasons for choosing a breast augmentation revision, the procedure can dramatically improve your breast appearance.

Everything You Need to Know About Modern Breast Implant Maintenance

shutterstock_278020754As of 2015, breast augmentation was the most popular cosmetic surgery performed in the U.S. 4 out of 5 implants used were silicon, and the other 1 out of 5 were saline. While breast augmentation has been highly beneficial for a wide age range of women, with any procedure, there are risks, and implants can have problems. Fortunately for most women, the implant process goes just fine, and they are left with beautiful results. To get a better understanding of breast implant preservation, we spoke with board-certified plastic surgeon, Dr. Frank Agullo of Southwest Plastic Surgery in El Paso.

Dr. Agullo, when should a patient have her breast implants replaced?

This will depend on the type of implant she has. Saline implants may deflate after a number of years, and will be ready for replacement at that time. Deflation is a fairly evident diagnosis, but if she has any suspicions about the implant, it’s important to speak with the plastic surgeon who performed the surgery. As long as the saline implant is unbroken, it is essentially perfectly functional and no replacement is needed.

Silicone implants are more popular, and the newer ones (from about 2006 to the present) have a lower rupture and capsular contracture rates. Capsular contracture is what we call the body’s response to a foreign object, such as a breast implant, by forming a scar tissue which then contracts (or tightens) around the foreign object. It can feel like a hardening of the breast and can occur with any type of implant. Silicone implants should be replaced if a hardening is felt, or in case of rupture.

Getting an MRI is the best way to check for any problems with your implant health, and should be done after the 3rd anniversary of your first implant surgery.

Despite not seeing any signs of problems or evidence of rupture, is it still a good idea to get them replaced eventually?

Through my years of experience, there seems to be no need to replace something that isn’t giving you any problems. And there’s no guarantee the new implants won’t have their own problems. If it’s not broke, don’t fix it.

Are there any types of implants that typically require more frequent replacement?

After 10 to 15 years, saline implants will probably leak at some point. And if the implant is unharmed, there’s no need for replacement unless the patient has rippling they’d like to fix, or a size change.

How do you feel about education available to women regarding their implant upkeep?

Implants usually require very little maintenance. Patients should be aware, though, that the best care for their breast health is done by getting MRI’s and having a post-surgery implant exam with their surgeon.

After the first year post-op visit, how often would you recommend women see their surgeon for an implant check-up?

Before and After Breast AugmentationAn annual checkup is what I recommend. Patients will usually pass this up and continue with their lives. But I will say that all women need an annual breast exam, especially those over 40, to ensure that they diagnose any form of breast cancer at an early stage.

Is there anything else you would like to tell women about their implant care?

Hardening can be very gradual, and many women won’t notice until it is very far along. There are non-surgical solutions when hardening is caught early. But routine follow-ups are the best way to insure your investment into this cosmetic surgery is wisely taken care of.

Remember, ladies, regular checkups are essential and always check your breasts for hardening or other concerns. It’s so important that while you look good on the outside, you are healthy on the inside.

Why You Need a Date With Your Plastic Surgeon – 10 Years Later

shutterstock_342061073You schedule regular check-ups with your physician, maintenance appointments for your car, and even get a job review on an annual basis – but what kind of follow up care do your breast implants need?

It’s great that everything looks and feels wonderful – but the important thing is that you should schedule a date with your plastic surgeon about 10 years after your breast augmentation has been done. After all, things aren’t quite what they used to be when you had the surgery a decade or more ago. Here’s why:

Your Body Changes

Even if you still look fit, feel young, and haven’t gained or lost a significant amount of weight – your body is changing. Things begin to droop, sag, or stretch whether we like it or not, thanks to gravity. Some women may find that things appear a bit uneven or more sunken than they were when they had their breast implants placed several years prior. Even your opinion about your breast size or shape may have changed. It could be time for something different when it comes to the perfect fit for your changing body.

A Safety Check

It’s natural for things to wear out over time. Even if you have a “warranty” on your breast implants, you still need to monitor them on a regular basis. Things can happen, such as accidental ruptures or leakage. If some form of abnormal swelling or misshaped anomaly occurs, the manufacturer of the implant usually only covers the replacement of the implant for less than 10 years. After that, you’re on your own. It’s beneficial to have Dr. Agullo check everything to be sure things look and function the way they’re supposed to.

As a matter of fact, Dr. Agullo recommends that you follow up with him every 6 months after your breast implant procedure. Of course, this is included with your procedure and there are no additional fees.

Implant Have Evolved

You may have “outgrown” the type of implant that you had placed a decade ago, and be ready to change it out with something better. Newer generations of silicone implants now offer our patients with a wider range of options when it comes to their breast size, shape, and appearance. From changing the shape of your breast and giving you a more natural feel, to lower complication rates – it doesn’t hurt to weigh your options against the older type of implant you’ve had for so long.

It’s Just a Healthy Thing to Do

Before and After Breast AugmentationThere’s no strict rule as to how long you need to go between your breast implant follow ups, but Dr. Agullo recommends scheduling a “date” at our office at least every 6 months or so, but definitely after 10 years. We want to be sure you’re pleased with your results and that nothing has changed in regard to your implant integrity, body contour or aesthetic needs. It’s not that we’re going to encourage you to change them out with something newer – it’s that we need to make sure you’re healthy, confident, and that things aren’t wearing out.
