What Happens During a BBL?
A Brazilian Butt Lift is a cosmetic surgery procedure that enhances the shape and volume of your buttocks using your own fat. During the procedure, excess fat is removed from “problem” areas (such as the abdomen, flanks, or thighs) using liposuction. The fat is processed in a centrifuge to remove impurities to use the highest-quality fat. This purified fat is then strategically injected into your buttocks to create a fuller, curvier behind.
Dr. Agullo uses the Viality™ fat transfer system and FDA-cleared surfactant wash Auraclens™ (containing Poloxamer 188) to clean, stabilize, and increase the viability of fat cells during autologous fat transfer (fat grafting). It improves fat survival rates to over 80%, providing better volume retention.
Week-by-Week BBL Recovery Timeline
Recovering from your BBL isn’t overly demanding, but it does take time, patience, and careful adherence to Dr. Agullo’s aftercare instructions. While every patient heals differently, the timeline below provides a general guide for what to expect at each stage of healing, helping you be better prepared and ensuring optimal results.
Week 1
The first week after your BBL is all about resting and protecting the newly transferred fat in your buttocks. Swelling, bruising, and soreness are normal during this stage, both in your buttocks and at the liposuction sites. You’ll wear a compression garment on the areas where you underwent liposuction to help minimize swelling and support your healing tissues. Most importantly, you must avoid sitting, lying, or placing direct pressure on your buttocks, as this can damage the transferred fat cells.
Dr. Agullo will provide you with his custom-made BBL pillow, set to keep pressure off of the buttocks when sitting or lying down. He will also start you on lymphatic massages with his certified therapist two or three days after your procedure. This is recommended at least two times per week for four weeks.
Week 2
Swelling and bruising typically begin to improve during the second week of your BBL recovery. You may start to feel more comfortable moving around and getting back to light daily activities. Dr. Agullo may allow brief periods of sitting with a specialized BBL pillow, which helps shift pressure from your buttocks to your thighs. However, do so only with explicit instructions and Dr. Agullo’s permission, as your results may be affected.
Weeks 3–4
By the third and fourth weeks following your BBL surgery, you will likely notice a significant improvement in swelling and overall comfort. Light exercise, such as walking, is encouraged to promote healthy circulation, and you may gradually resume your daily routine. You can begin sitting directly on your buttocks at this point, however, prolonged sitting and strenuous exercise should still be avoided.
Weeks 5–6
Around this stage in your BBL recovery, you may be cleared to sit for longer periods using a BBL pillow. Swelling continues to resolve, and your results become more visible. Your buttocks begin to take on their noticeably enhanced shape as your body adjusts to its new contours and the transferred fat settles in.
Months 2–3
By two to three months after your BBL surgery, most of the healing process is complete. The fat cells that have successfully established a viable blood supply will become a permanent part of your body, creating long-lasting volume and improved contours.
What Affects Your BBL Recovery Timeline?
Several factors can influence how quickly and comfortably the recovery process is after your Brazilian Butt Lift, including:
- Whether you had additional cosmetic procedures: Combining your BBL surgery with procedures like body contouring or a supercharged BBL may extend your recovery period.
- Your age and overall health: Patients who are in good general health often heal more efficiently.
- Following your surgeon’s instructions: Avoiding smoking, staying well-hydrated, wearing compression garments, and adhering to activity restrictions all help ensure a smoother recovery.
Dos and Don’ts of BBL Recovery
Proper aftercare is essential for preserving your BBL results and helping the transferred fat cells establish themselves. Some important tips to remember include:
Do:
- Wear your compression garment as instructed.
- Sleep on your stomach or side.
- Stay hydrated and maintain a healthy diet.
- Take short walks to encourage circulation.
- Use a BBL pillow when sitting is permitted.
Don’t:
- Sit directly on your buttocks during early recovery.
- Smoke or vape, as nicotine reduces blood flow and can harm fat survival and slow healing.
- Engage in strenuous exercise too soon.
- Skip follow-up appointments.
- Wear tight clothing that compresses your buttocks.
FAQs About BBL Recovery
What position can I sleep in after my BBL?
After a Brazilian Butt Lift, you will need to sleep on your stomach or side for several weeks. These positions prevent excessive pressure from being placed on the newly transferred fat cells, allowing them time to establish a healthy blood supply and improve their chances of long-term survival.
When can I sit after my BBL?
Dr. Agullo typically advises his patients to avoid sitting directly on their buttocks for at least two weeks after surgery. After that, he may allow limited sitting using a specialized BBL pillow that distributes weight to your thighs, keeping pressure off of your buttocks.
When can I get back to the gym after my BBL?
Light walking is encouraged almost immediately after your BBL surgery to boost circulation. However, strenuous workouts, weight training, and high-impact exercise should be avoided for four to six weeks, or until Dr. Agullo clears you to resume them.
