The Pros and Cons of Breast Augmentation With Fat Transfer

lady-measuring-her-breastsIn all honesty, there are advantages and disadvantages to virtually all cosmetic procedures. However, the true list of pros and cons is completely determined by the patient and will be different for every individual; an ideal procedure for one person may not be the best option for another. Here are the facts about breast augmentation with fat transfer to help you determine your particular list of pros and cons:

It Reduces Body Fat

Perhaps the best and most obvious advantage of breast augmentation with fat transfer is the loss of fat from other areas of the body. The donor area can be the thighs, hips, buttocks, or abdomen—almost any larger area of the body that has excess, unwanted fat. Patients experience a few inches lost from the particular area and can feel more confident in smaller or tighter clothing.

It Is Less Invasive Than Implant Placement

Breast augmentation with fat transfer is less invasive than implant breast augmentation depending on the amount of fat removed during the liposuction portion of the procedure. Liposuction will require only a few small incisions at the donor site but may result in swelling and bruising that will cause some discomfort during recovery. The surgical step of injecting the fat into the breasts is much less invasive than surgically inserting implants. Generally, the fat grafting procedure results in a shorter and easier recovery than implant augmentation.

It Has Lower Risks Than Implant Augmentation

With breast implants, patients run the risk of capsular contracture, implant malposition, rippling, and rupture. With fat transfer to the breasts, the patient avoids all implant-associated risks. The body also will not reject the patient’s own fat as a result of fat transfer.

The Breasts Look and Feel More Natural

Because the patient’s own fat is used, the breasts will look and feel more natural compared to silicone or saline breast implants. For those who would like more defined roundness and cleavage at the upper pole of the breasts, breast implants may be a better option.

When fat is injected into the body, it must form a blood supply to survive. Only about 50 to 70 percent of the injected fat is likely to survive in the breasts, which can lead to less dramatic results than with breast implants. After four to six weeks, the size of the breasts stabilizes. As long as you maintain your weight, the breast size will remain the same. If you gain weight, the breasts will increase in size. They will decrease in size if you lose weight.

It Provides a Less Significant Increase in Breast Size

Patients should expect an increase of about one to two cup sizes with fat transfer, while breast implants provide many more size options. Patients can choose to get more than one fat injection treatment to achieve the size they want, but this option often takes longer than breast implant surgery and may end up costing more as well.

Misconceptions About Breast Augmentation and Lifts

shutterstock_256814590There are many reasons why some women may want to undergo augmentation mastopexy, or a breast lift – a procedure where excessive skin around the breasts is removed, to tighten and contour the overall chest area.

The results of a breast lift improve erase concerns such as stretching or drooping that commonly occur after a woman has had children, breastfed, lost weight, or aged. I typically recommend it for my patients that are experiencing sagging and reduction in the fullness of their breasts, along with procedures like breast implants. Yet, there are still a lot of misunderstandings surrounding this procedure – and many women wonder if it is something that they’ll actually need.

Common Misconceptions About Breast Lifts

There are two misconceptions that we frequently hear our patients bring up, and it is critical to clarify the information around these before any patient undergoes treatment here at Southwest Plastic Surgery. The first misconception I hear is, “I’ll only need one surgery, because I’m getting my lift and augmentation done simultaneously.”

In reality, this process involves two different processes to make the breast larger in the areas requiring fullness and smaller in the areas experiencing sagging. For some, not all patients, it is safest to have a series of procedures instead of forcing it into just one. This depends on the level of sagging, and also allows the body to heal itself on a more gradual scale.  Having a series of procedures can reduce risk of complications, such as reduced nipple sensation.

Before and After Breast LiftThe second misunderstanding regards scarring. “From what I hear, it’s just supposed to be an incision around the areola and nipple area, so scarring won’t happen.” Each person is different, which means their breasts and skin are different. Plastic surgery will always leave some type of scar; they can be minimal, or more prevalent. This is the trade off of cosmetic procedures: getting a scar, or not having the procedure done. Individuals may have their incisions in different locations than expected, but that can be discussed prior to surgery. Patients with more sagging will, generally, require more incisions to create a breast lift. Less sagging, on the other hand, will require fewer incision sites.

Everything You Need to Know About Modern Breast Implant Maintenance

shutterstock_278020754As of 2015, breast augmentation was the most popular cosmetic surgery performed in the U.S. 4 out of 5 implants used were silicon, and the other 1 out of 5 were saline. While breast augmentation has been highly beneficial for a wide age range of women, with any procedure, there are risks, and implants can have problems. Fortunately for most women, the implant process goes just fine, and they are left with beautiful results. To get a better understanding of breast implant preservation, we spoke with board-certified plastic surgeon, Dr. Frank Agullo of Southwest Plastic Surgery in El Paso.

Dr. Agullo, when should a patient have her breast implants replaced?

This will depend on the type of implant she has. Saline implants may deflate after a number of years, and will be ready for replacement at that time. Deflation is a fairly evident diagnosis, but if she has any suspicions about the implant, it’s important to speak with the plastic surgeon who performed the surgery. As long as the saline implant is unbroken, it is essentially perfectly functional and no replacement is needed.

Silicone implants are more popular, and the newer ones (from about 2006 to the present) have a lower rupture and capsular contracture rates. Capsular contracture is what we call the body’s response to a foreign object, such as a breast implant, by forming a scar tissue which then contracts (or tightens) around the foreign object. It can feel like a hardening of the breast and can occur with any type of implant. Silicone implants should be replaced if a hardening is felt, or in case of rupture.

Getting an MRI is the best way to check for any problems with your implant health, and should be done after the 3rd anniversary of your first implant surgery.

Despite not seeing any signs of problems or evidence of rupture, is it still a good idea to get them replaced eventually?

Through my years of experience, there seems to be no need to replace something that isn’t giving you any problems. And there’s no guarantee the new implants won’t have their own problems. If it’s not broke, don’t fix it.

Are there any types of implants that typically require more frequent replacement?

After 10 to 15 years, saline implants will probably leak at some point. And if the implant is unharmed, there’s no need for replacement unless the patient has rippling they’d like to fix, or a size change.

How do you feel about education available to women regarding their implant upkeep?

Implants usually require very little maintenance. Patients should be aware, though, that the best care for their breast health is done by getting MRI’s and having a post-surgery implant exam with their surgeon.

After the first year post-op visit, how often would you recommend women see their surgeon for an implant check-up?

Before and After Breast AugmentationAn annual checkup is what I recommend. Patients will usually pass this up and continue with their lives. But I will say that all women need an annual breast exam, especially those over 40, to ensure that they diagnose any form of breast cancer at an early stage.

Is there anything else you would like to tell women about their implant care?

Hardening can be very gradual, and many women won’t notice until it is very far along. There are non-surgical solutions when hardening is caught early. But routine follow-ups are the best way to insure your investment into this cosmetic surgery is wisely taken care of.

Remember, ladies, regular checkups are essential and always check your breasts for hardening or other concerns. It’s so important that while you look good on the outside, you are healthy on the inside.

Is it Time to Trade in Your Breast Implants?

With all of the latest technological advancements in breast augmentation surgery and implants, many women are opting for the newest methods of augmentation. A great number of them are having their second, or even third, augmentation procedure.  Why? Because they want to take advantage of the newer shapes and different material from their original implants.

shutterstock_352067231The desire to change from their current implants (which may be perfectly fine) in order to utilize modern advancements to enhance their appearance is what we refer to as elective implant exchange.

Elective Implant Exchange in El Paso

Elective implant exchange procedures are most popular with those who have had augmentation within the last decade, as the technology behind the process has made the most advancement just within the past few years.

A change in material, size, and shape are what most women desire when they undergo the operation. A woman could switch to saline or silicone, a B to a D cup, or from a round shape to a more natural teardrop shape – just to give you a few ideas.

Gummy Bear Implant Options

The newest silicone inserts come in what has become known as “gummy bear” implants, due to their consistency being similar to the squishy candy. They’ve become highly desirable, as they are available in the new teardrop shape, which looks more anatomically correct. In fact, it has become the most requested item here in my El Paso office when a procedure is done to switch from saline implants, which are more prone to rippling, wrinkling, and leaking.

Voluma Silicone Implants

One of the implants I am most excited about are the new Voluma Natrelle implants by Allergan. These are similar to previous models but they have more filling within the outer shell. By doing this, they achieve a fuller, rounder breast with much less rippling.

Beware of Buyer’s Remorse

There are, of course, many reasons why a woman may want to switch out her implants. One of the more prevalent reasons is “buyer’s remorse”. While there does exist a clientele who will downsize their bust, the general population of this client segment is patients who wish they’d gotten a larger size implant from the very start. Besides a change in shape, size, or material, the lifespan of the implant itself plays an important role in having them replaced.

Follow Up is Important

Before and After Breast AugmentationOf course, implants are not considered to be lifelong devices. Annual checkups and MRI’s are crucial to maintain breast health and can reveal when it is time to have a replacement or other care procedure performed. There has been a slight decrease in breast augmentation procedures for new patients, but repeat patients who are having replacements done has increased significantly across the country.

The best idea is to be true to what you want from your implants. Trends will change with plastic surgery advancements, as with anything else. Larger breasts trended several years ago, and now seeming more natural, smaller, and streamlined is the ideal. Going with what you and your surgeon see as the best fit for your body will leave you with the best results, and happy until the time may come when your first pair of implants deserve retirement.

Is 2016 the Year for a Trend in Smaller Breast Size?

shutterstock_272854151D-size breast implants may not be the latest trend this year. A survey of plastic surgeons showed that we’re actually seeing a trend in smaller, more naturally sized breasts.

Why? Likely because women want their breast augmentation process to look as realistic as possible. Women are opting for less-full chests, with saline implants now being filled on an average of about 234 cc’s.

Are Alternatives Better for Less Noticeable Differences?

Store bought pills and pumps won’t work when it comes to enlarging the breast tissue. One easy method that some women consider though is the “Instabreast” technique. Basically, it involves a small amount of sterile saline that is injected into the breast to offer you temporary enlargement. That way women can sort of try them out before they move forward with an actual surgical procedure. It’s not something that will last for very long, but it does allow for better visuals if you need a bit more time to consider what size is best.

Why are Women Opting for Smaller Chests?

If a woman is going to go through with having her breasts enlarged, the past way of thinking would have been something along the lines of “make it worth it,” that is, jumping several cup sizes up.

Now, ladies want something that’s noticeable yet natural looking at the same time. If it’s something they can’t get with just a bit of a padded bra under their blouse, they’re considering conservative sized breast implants.

“Test Drive” Your New Breast Size First

But how can we make sure we’re not placing a breast implant that’s too small? It’s simple – using the Instabreast procedure for a real-life “test drive” of their new breast size before augmentation surgery. Very small amounts of saline are placed into the breast tissue (don’t worry – it’s completely safe!) so that women can see the size differences in real life before the surgery date. If the breasts are still too small, we can add more saline until we get it just right.

Utilizing the Instabreast procedure saves women from wanting to go back after augmentation surgery to revise the size of their implant. If you’re not comfortable with someone else’s recommendations when it comes to size, I feel that the Instabreast procedure is the perfect way to be sure you’re moving forward in the right direction.

Why are Women Over 50 Turning to Plastic Surgery?

Before and After Eyelift SurgeryWomen in their 50’s are increasingly turning to plastic surgery, and the reason why might surprise you. New financial freedoms mean that if you’re in your mid to late 50’s, you can spend your money where you want to – whether it’s traveling the world, doing some extra shopping, or even purchasing a new home.

A lot of women are using their pensions and investment incomes on things they’ve always wanted – like plastic surgery. More and more 50 and 60-somethings are coming in for cosmetic procedures like breast implants, body recontouring, and facial aesthetics (like eye lifts or Botox.)

Mature Patients Make Up the Majority

In 2015, the American Society for Plastic Surgeons (ASPS) said that individuals age 55 or over made up nearly 1/4 of all of the patients seen for cosmetic treatments. Not surprisingly, those in the age bracket of 40-54 made up over 1/3 of all patients. This can come as a surprise to most people, who assume cosmetic surgery is only for the young 20 or 30-something year olds.

Why the huge increase? It could be that more mature women are choosing to “tweak” their appearance, rather than change it completely. Subtle, age-defying procedures like eye lifts or injectables make it less obvious you’ve had any “work” done. More dramatic procedures like nose jobs are becoming less common among women in these age brackets.

Financial Freedom to Invest in Yourself

Being that people over 55 are free from a lot of the financial obligations that they had as younger adults (such as purchasing a home or raising a family,) they are often free to enjoy a bit more flexibility with their spending. Add in the fact that cosmetic treatment’s aren’t really regarded as a “taboo” thing anymore, people that might not have considered getting work done now find themselves moving forward with minor or less invasive procedures. They’re often surprised to see that cosmetic surgery is actually quite affordable for the individual on an average-income.

What Are People Getting Done?

Women are coming in for injectables or other types of non-surgical procedures to add youth and volume to their faces. They just want to feel vibrant and less tired when they look in the mirror. Since people are living longer, we don’t want to look older than the age that we feel!

While more invasive surgeries are usually left to the younger crowd, 50-somethings are not hesitant one bit to take advantage of simple outpatient or non-surgical treatments. The women that are going in for breast augmentation are typically those that had the surgery done a decade or so ago, and want to change them up a bit to fit their new body.

It’s Not Just Women! Men are Getting in on the Action, too!

And women aren’t the only ones taking advantage of convenient treatments. Men are joining in too! In most cases, men just want to look younger or less tired, and there’s a lot we can do to make that happen. Some men choose to have things done to their neck or eyes, to gently lift the skin and add a youthful glow to their appearance. Some of them are even getting beard implants!
