October 17, 2026 | Buttock Augmentation, Male Procedures, Plastic Surgery News

Allure asked five plastic surgeons whether “glutemaxxing” has reached the operating room. The piece, by Elizabeth Gulino on October 13 (Allure), lands where I do: men want projection and an athletic shape, not an hourglass, and the ones who come in are asking about donor fat more than a BBL.
I was not one of the five. I am one of the surgeons doing it.
“We’ve been using alloClae very often. I’m one of the early adopters, and actually one of the first 20 surgeons who was invited to start using alloClae, more than a year ago. The most common places we’re using it are in the breast, in liposuction contour irregularities, and in the buttock, especially for projection or to fill in the hip dips. We’re performing this procedure at least two to four times per week, so it has become a very popular therapy.”
What the Numbers Say
The American Society of Plastic Surgeons counted 550 Brazilian butt lifts in men in 2025, up 48 percent from 373 the year before, against more than 25,000 in women. Injectables like alloClae are not in that count. Two of Allure’s five surgeons said they have seen no real uptick in men, and one said far fewer men go through with surgery than talk about it. Fair. Most of the hashtag is squats.
Why Men Do Not Want the BBL Shape
“Also, when we do buttock fat grafting in men, we usually skip fat grafting to the hips, because we want to avoid a feminine curvature and instead project the gluteus maximus muscle.”
That one sentence is the difference between a BBL and what a man is asking for.
The Patient Who Picked Donor Fat Over a BBL
“The first patient, highlighted in the story, actually had enough fat in the flank area, so we could have done a BBL. But he is a professional who did not have any downtime and wanted to get back to work the next day. He preferred alloClae, since it is done with local anesthesia and he can return to work the next day and to exercise within a week. He favored this type of procedure and liked it so much that he repeated it a few months later for further volume.”
Local anesthesia. Work the next day. Gym in a week. That is the glutemaxxing patient.
The Lean Patient With Nothing to Give
“The second type of patient is usually someone who has already had multiple BBLs and liposuction procedures, so there truly isn’t any donor fat the patient can provide from their own body. At this point, we have to rely on biomodulators like Sculptra. But as soon as alloClae came out, it became a much better option, since it feels like fat, tends to last much, much longer, and is more predictable.”
Ozempic Butt in Men
“The other thing I am seeing often is that the patient feels better and is healthier, but the body, with the loose skin, is not showing that externally. So many men are coming in for gynecomastia and tummy tucks, and some men are coming in to restore the volume they have lost in the buttock, where we do liposuction and fat grafting.”
When It Is an Implant, Not Fat
“Implants I really recommend if what you’re looking for is more projection. It does give us better projection than fat. With the implant we can really tell how much you’re going to get in projection, because it will pop. Fat is a little bit squishier, so it doesn’t pop as much. But it does give you a nice shape.”
“Later on, once you’re done with everything, if you still need a little more volume, there is something called allograft, which is a donor fat that we can inject even in the office. You don’t have to go under general anesthesia, and it can fill in that area just to give you a little bit more symmetry.”
The Honest Line on Donor Fat
Allure notes scattered reports of lumps and cysts with alloClae, mostly in the breast, and limited human data. I tell patients the same thing I tell them about the regulatory question.
“The difference here is that donor tissue does not go through the normal FDA approval protocols like drugs and medications do. Instead, the process used to prepare the donor tissue must go through FDA guidelines. Therefore, alloClae complies with all guidelines necessary to be used in a medical fashion. alloClae did not skip FDA approval, because donor grafts do not go through the same FDA channel as drugs. They do have to comply with strict FDA guidelines.”
