Internal Spanx Surgery for the Buttock

beautiful-buttocksI was recently interviewed by NewBeauty Magazine in regards to a new procedure that is known as Internal Spanx Surgery for the buttock. It was developed in Brazil and involves using Polytetrafluoroethylene mesh to lift the butt area. Currently, the procedure is being performed in Brazil and Portugal. You can read the full article in the NewBeauty blog. Here are the questions I was asked:

1.     Is Polytetrafluoroethylene mesh safe to use in the buttock area?

There’s not enough data or long term follow up to say that the use of the mesh in the buttocks is safe. This is a new procedure. Although, there have been case reports of the use of mesh and even threads in the past to provide a less invasive buttock lift.

Mesh has been successfully used by others and myself to provide an internal silk bra (I have presented this in the past and will be presenting it in Japan next Wednesday at the ISAPS, International Society of Aesthetic Plastic Surgery, Meeting) with little if any complications. It has also been used safely in the abdomen to provide a permanent internal girdle during tummy tucks.

2.     What are some risks associated with a procedure like this?

The most concerning risk is that the mesh can become infected, which would require removal. Other concerns would be technique specific: asymmetry and palpability of the mesh.

3.     Are there any limitations or drawbacks to having a procedure like this instead of a Brazilian butt lift or butt implants?

Each of the procedures you mention is indicated for different purposes. I believe each procedure may have its place for individual patients. This Internal Spanx procedure seems to provide a means to achieve a less invasive lift with mesh. It would likely be indicated in a thin patient with some buttock ptosis (drooping) but no excess skin, and no desire for increased fullness.

A more traditional buttock lift entails an incision similar to a g-string above the buttocks, which removes excess skin and even fills the buttocks using the patient’s own tissue.

For the patient looking for accentuated hips and buttock the likes of Kim Kardashian, Jennifer Lopez or Nicki Minaj, you have to introduce volume which can only be achieved with fat transfer to the buttocks (Brazilian butt lift) or implants. The added benefit of a BBL is that the fat is removed from unwanted areas to create a small waist and transition to buttocks and hips. In thin patients without excess fatty deposits, the only option is to use implants.

4.     What are your thoughts about the safety and effectiveness of this procedure?

The use of the Polytetrafluoroethylene mesh is off-label in the United States. Since it is a new procedure, long-term data and results are necessary to comment on the safety. It is always fascinating to see new procedures and techniques developed!

5.     What would you recommend to a patient who was considering this procedure?

Seek a board certified plastic surgeon specializing in buttock procedures to consult on which procedure is ideal for you. Many surgeons discourage patients from bringing wish pictures, but for the buttocks, this is one of the best ways to convey what you want.

What You Need to Know About Getting a Bigger Butt

In Miami, one woman spent over $10,000 to have a rear that resembled that of the socialite star. Before that, she was doing 300 squats a day! Ultimately, a Brazilian Butt Lift was her only option to get the round rear she wanted – so she went the surgical route.

THREE BUTTS IN THONGSThe American Society of Plastic Surgeons has released new data today, the 2014 statistics report.  The data showed that two of the fastest growing procedures in the United States are buttock augmentations with implants and buttock lifts.  In 2014, 5,863 buttock implants were performed, and 3,505 butt lift procedures were done during the same time.   This represents a 98% and 44% increase respectively.  At the same time buttock augmentation with fat grafting, also known as Brazilian butt lift or buttock fat injections, increased 15%.  This complete  combination of procedures is only offered by a few plastic surgeons within the United States.

Brazilian Butt Lift (BBL) and fat grafting have increased throughout the years mainly due to an increased interest by women for the buttock contours made popular by celebrities such as Kim Kardasian, Jennifer Lopez, and Nicki Minaj.  The popularity has also increased with the recent explosion of #belfies, or “butt selfies.”  Many women are trying to achieve and improve buttock contour, but not all are good candidates for only a Brazilian Butt Lift or fat grafting.   A Brazilian Butt Lift incorporates liposuctioning fat from areas of undesired deposits and transferring them to the buttocks.  This results in a smaller waist size, thinner thighs, and even flatter abdomen when transferring this fat to the buttocks, creating more voluptuous curves.

Frustrated by the trends, many women that are in shape and do not have a large amount of excess fatty deposits, have turned to buttock augmentation with implants.   Although buttock augmentation with implants has had a bad rap on the Internet mostly due to substandard procedures and inexperienced physicians, many have found benefits and good results from the procedure.   The main emphasis is placing the implants within the muscle thus avoiding common complications such as implant palpability, ability to move the implant and rotate it, buttock ptosis, double bubble deformity and seromas.  The implants used in buttock augmentation are solid silicone implants; therefore they never break and should last a lifetime.  Furthermore, by placing the implant within the muscle, the implant will never droop or sag.  Seromas and implant palpability with implants within the muscle are almost unheard of.  Buttock augmentation with implants is truly the only option for a thin woman in shape who desires improved buttock projection and contour.

Some women experience buttock droopiness or sagginess.  In order to address this issue a buttock lift is often the only solution.   A buttock lift is performed by making a G-string type incision above the buttocks where the skin can be tightened and lifted.   The same tissue that is usually excised for a buttock lift can be used for augmentation.   In some patients a buttock implant or even buttock fat injections can be combined for buttock lift to provide the ultimate result.    Although many women are concerned with the scarring left behind after a buttock lift, you can rest assured that the incision will lay so that it can be covered by a G-string, bikini, or underwear.

#Belfies Are the Latest Social Media Rage

But if you haven’t heard of “Belfies”, these are the latest trend to hit social media sites like Instagram, Facebook, Twitter, Pinterest and even YouTube. top listThe results are in, and NBC recently released the results of a poll taken from 953 doctors across the United States. It seems that breast augmentations, while still wildly popular, have taken a “backseat” to these two other plastic surgery procedures as the most asked for. Southwest Plastic Surgery of El Paso, TX is one of the few places to offer the services, which have both doubled nationwide.

What are they?

Oddly, enough, America has moved its interest in the chest to the backside, with butt implants nearly doubling in requests over the tally in 2013. Brazilian Buttlifts were also very popular, with a 15% surge in patients receiving them over the prior year. NBC News did an interview with the president of the American Society of Plastic Surgeons, who explained that “butts are in”, perhaps in part because of the huge infatuation with Kim Kardashian and the media craze.

Their findings were also in line with the American Society for Aesthetic Plastic Surgery, whose survey of 901 doctors found an even greater increase of both the silicone butt implants and the Brazilian Butt Lift, in which fat injections remove the fat from one area of the body and reposition it to the derriere. The president of that organization cited social media as one of the instigators behind this, which affirmed the findings of the ASPS.

Along with the popularity of the Brazilian Butt Lift and the silicone implants, now more women are asking for another highly personal cosmetic procedure. Labiaplasty has also grown in popularity, making it the second fastest growing procedure in 2014 after the butt surgeries. Buttocks augmentation procedures work in three different ways to achieve similar endpoints. It can be achieved through fat injections in combination with liposuction, with gluteal implants, or with your own tissue if you need a lift at the same time.

The “Brazilian Butt Lift”, is achieved by removing fat from a patient through liposuction and then injecting or “repositioning” the fat into the buttocks for reshaping. By taking a person’s own fat, this relatively simple cosmetic enhancement can have dramatically pleasing results by transforming a flat butt into a wonderfully shaped, round butt that sits higher and looks more appealing in bathing suits or form-fitting apparel.

Dr. Agullo also performs buttock augmentation with silicone implants. The use of implants can generate more dramatic results and is a good option in thin patients that have little fat for injections.

Buttock Augmentation Techniques

Buttock augmentation has a few different terms associated with the procedure, aside from the “Brazilian Butt Lift”. It can also be termed butt implant, gluteal implant, buttock enlargement or just a ‘butt lift’. These all refer to procedures used to improve the buttock contour, although the techniques may differ.

During buttocks augmentation surgery, you will be under general anesthesia or IV sedation:

  • Buttock Augmentation with Fat injections. This is one of the most common methods of buttock augmentation also termed “Brazilian Buttock Lift” or BBL. It works best for patients who have excess fat stored in other areas of the body, such as the thighs, abdominal (midsection), hips and waistline. Dr. Agullo uses a process called fat grafting or fat transplantation, in which he will gently remove the fatty accumulation from these areas with liposuction and then reposition them strategically within your butt to achieve a naturally fuller, rounder shape. Liposuction is always performed above the buttocks, which improves the overall contour.
  • Buttock Augmentation with Implants. Patients who have only minimal body fat and who want the buttocks augmentation enhancement will most likely need implants to achieve their goal of a round or more projected butt, which involves the plastic surgeon placing a strategic incision just below the tailbone. The incision is not visible when standing, as it hides in the fold. Dr. Agullo’s preferred implant placement is intramuscular, which avoids many of the complications previously associated with buttock implants. Buttock implants are made of solid silicone gel, so they do not pose a risk of rupture or leakage.
  • Buttock lift with augmentation. If you have buttock ptosis, then a lift may be necessary to achieve the best gluteal contour possible. In mild cases of ptosis adding volume with fat injections or an implant may be enough. In more pronounced cases a buttock lift would be necessary. A buttock lift is achieved by removing skin just above the buttocks, resulting in a thong type of incision. The scar is easily hidden with underwear, bathing suit, or even thong type undergarments. Augmentation of the buttocks is achieved at the same time with a local flap from your own fatty tissue. In certain cases augmentation can also be achieved with fat injections or implants.

No matter what you call it, buttocks augmentation can be done as part of other cosmetic procedures like a tummy tuck and breast lift or with an entire body lift experience. Your buttocks augmentation will be done in Dr. Agullo’s state-of-the art facility most likely as part of an outpatient procedure, which dependent upon whether the butt lift is combined with other cosmetic applications can take about 2 hours.

Getting ready for butt lift surgery

Get ready to have your buttocks lifted, contoured and protruding just the right way with Southwestern Texas buttock augmentation surgery! Here are some readiness strategies to apply before your buttock enlargement procedure:

  • Quit smoking any tobacco products for at least two weeks before surgery and two weeks afterwards to decrease potential risks from blood clots or slower.
  • Line up relative or friend to take you to and from the procedure. You won’t be able to drive after having anesthesia for at least a few days afterward.
  • Plan to get help with childcare, errands, etc. since you won’t be able to lift or do heavy chores.
  • Make sure you don’t eat or drink anything after midnight on the evening before your buttock augmentation procedure.
  • Avoid using cream or lotions on your skin in the derriere because it will block pores.
  • Wear something comfortable that can be easily slipped on and off. Avoid anything with buttons, zippers or that is too tight. Sweatpants, leggings, skirts or shorts will be best.

Combining buttock augmentation with other procedures

Sometimes buttock augmentation surgery can be combined with other cosmetic procedures, such as part of a body-sculpting package or in conjunction with anabdominoplasty (tummy tuck). Recovery from buttock augmentation

After the buttock augmentation, you will need to wear a special compression garment that is designed to keep everything “tight” and help it heal properly. A partial absorption of fat that was transferred will settle after about three weeks. You should wait at least three months to decide whether future grafting is needed or if you are satisfied with the full effects. Try to sit as infrequently as possible for the first few weeks to reduce absorption and soreness to your buttocks.

Other things to know about recovering from butt augmentation:

  • Recovering from buttock augmentation requires mostly laying down or standing for at least a couple of weeks after the surgery. You won’t be able to sit down right away and may feel sore or tender when you do.
  • After two weeks you should be able to resume work and normal activities.
  • You should walk around some to avoid blood clots; however you should avoid major exercise for at least a month.
  • Tell your doctor about any chest pain, abnormalities in heart rate, pain, or shortness of breath.
  • Don’t take any aspirin.
