A patient I have treated for the better part of a decade walked into the office last month carrying a small insulated cooler. Inside was a vial of donor-derived exosomes she had bought through a friend of a friend at a wellness clinic up the highway. She wanted to know, in her words, “if this is the real thing or a $1200 facial in a box.”
I asked if I could see the vial. I read the label. There were no payload concentrations listed. No identification of which growth factors were measured, in what amounts, or whether the supernatant had been filtered out. The dose count was “five trillion exosomes.” That number, in isolation, tells a clinician nothing.
I told her two things. First, I would not put what was in that vial through a patient’s microchannels. Second, the regenerative aesthetics conversation has actually moved past it, and I wanted to walk her through where it has gone.
This is that walkthrough. The product at the center of it is called YOU by Acorn, and it is the first autologous, stem-cell-derived secretome treatment I have brought into my practice. The rest of this piece is the clinical version of why.
What Secretome Means, in Plain English
Stem cells are not, in the body, factories that turn into the tissue you want them to turn into. That was the first generation of stem cell hype, and it was largely wrong. What stem cells actually do, when alive and in contact with tissue, is screen the surrounding environment and secrete the signals that surrounding tissue needs to repair, regenerate, and stay healthy.
The secretome is the output of that screening. Growth factors. Cytokines. Collagens. Proteins. Exosomes (the carriers). All in the proportions your own biology produces them, not in proportions a manufacturer guessed at.
Worth being explicit, because patients keep asking. The bottle is the secretome. The bottle does not contain stem cells. The cells are banked separately, cryopreserved as the patient’s own property. The product applied to the face or scalp is everything those cells produced when the lab put them to work, not the cells themselves.
Autologous means the source is you. There is no donor. There is no risk of immune rejection, because everything in the bottle is made from cells your body already recognizes as yours.
YOU by Acorn is, to my knowledge, the first commercially available autologous secretome serum in the United States made from a patient’s own mesenchymal stem cells. The cells are not collected from your iliac crest (painful) or from liposuctioned adipose tissue (hard to isolate, oily). They are collected from the base of your hair follicles. Pluck fifty hairs from the back of the scalp, and there are roughly 1,500 to 4,500 mesenchymal stem cells at the base of each follicle. The lab needs about 27 viable follicles to bank a patient. Fifty gives them margin.
What Is in the Bottle
When the kit comes back to the clinic, you receive twelve vials of 2cc each. The lab also offers a six-vial entry option for patients who do not need the full course at first.
The published characterizations Acorn shares with prescribing clinicians:
- Approximately 5 billion exosomes per vial
- Approximately 34 times more concentrated than the best platelet-rich plasma you could draw on a healthy day
- Stable refrigerated for 18 months from the manufacturing date
- No preservatives
- FDA-cleared use today: topical application onto skin after microchannels are made by microneedling, Morpheus8 RF microneedling, or fractional laser resurfacing, and topical application onto the scalp after microneedling for hair
That is the entire FDA-cleared use case as of this writing. YOU is not an injectable. The window we care about is the brief few hours after a procedure has opened thousands of microchannels into the dermis. That is when the secretome reaches the compartment that is asking for it.
Why I No Longer Use Generic Exosomes
For the last two years, my message to patients on donor-derived exosome bottles has been “I have not adopted these.” I have left it at that publicly. With the launch of YOU by Acorn I will go further.
An exosome is a delivery carrier. It is, on its own, an empty Easter egg unless someone has loaded it and assayed the load. The donor exosome industry typically cultures a stem cell line, harvests the conditioned media, and then filters out everything but the exosomes themselves before bottling. Whatever signal lived in the supernatant is discarded. What gets shipped is a carrier population whose payload is rarely measured and never personalized.
YOU by Acorn keeps the exosomes and the surrounding signal. The 5 billion exosomes per vial sit in the cytokine and protein milieu the cells generated when they were stressed in the lab. You receive both the carriers and the cargo, and Acorn shares the assay data on what is in there. That is a different conversation from the one I have been having about generic exosomes, and it is the reason I changed my position.
How YOU by Acorn Compares to PRP, PRF, and Recombinant Pure PDGF

The choice between recombinant pure PDGF and YOU by Acorn is not either-or in my practice. Pure PDGF is a single, controlled-dose growth factor that pairs cleanly with one specific procedure. YOU by Acorn is a broad, autologous signaling profile that pairs with multiple procedures over multiple visits, and that can be replenished from the same banked cells for the rest of the patient’s life.
Workflow Inside My MedSpa
The collection takes a few minutes. We move around the back of the scalp, where nerve density is forgiving, and pluck fifty intact hairs with their roots. Patients have described it as the least painful thing that happens inside our walls, which is fair. The follicles ship to Acorn’s lab in California for viability check (the company is headquartered in Canada; the lab work happens stateside). One quarter of the cells are cryopreserved on arrival. The rest go into culture for fifteen days to produce the secretome.
Roughly 45 days later the kit arrives at the clinic. Twelve vials, 2cc each, refrigerated. For a face treatment we run a Morpheus8 RF microneedling, microneedling, or fractional laser session, then immediately reconstitute two vials with the topical-grade hyaluronic acid that ships with the kit (do not use that HA for anything other than topical work, by the way) and apply the solution into the freshly opened channels. Patients can take a small dropper home and continue topical application twice a day while their channels are still open, which materially shortens the visible recovery window. The patient case I keep showing other physicians is a woman who had a CO2 treatment two years ago and a single secretome-paired microneedling round more recently. The day-one through day-five photos in the second course look like a different patient.
For hair, we are using YOU for Hair as a microneedled scalp treatment in series. Patients already invested in PRF or PRP for hair do not need to choose between regimens. We layer YOU into their existing schedule for one or two sessions, and we let them tell us what they prefer once they have seen both.
Banking, on Principle
Every collection cryopreserves one quarter of the patient’s stem cells. The patient owns those cells. The annual storage cost is modest. The implications are larger than most patients realize at first.
Acorn’s lab can take a banked mesenchymal stem cell, induce it back into a pluripotent state, and from there derive lineages that include bone, cartilage, pancreatic cells, neurons, and natural killer cells. The company’s founder, Drew Taylor, has demonstrated this with his own brother’s cells, deriving insulin-producing pancreatic cells from hair follicle stem cells in the lab. The clinical translation of that work is years away. The bank is already useful for the patient who wants to make their cells available, with their consent, to future therapies derived from their own biology.
You are also banking the version of you who walked into the room today. Every secretome run for the rest of your life will be made from cells frozen at that age. That is the longevity claim, and it is a defensible one.
What I Will Not Say About This Product
I will not tell you it reverses the biological clock. I will not promise outcomes I cannot defend with the data Acorn has so far. I will not present this as a replacement for sunscreen, retinoids, your existing facial program, or for the procedure itself. I will not endorse off-label uses outside of what the FDA has cleared.
What I will tell you is what I observed on my own face. I scheduled a Morpheus8 RF microneedling session in my own clinic and microneedled two cc of YOU by Acorn into the freshly channeled tissue. One week later my wife noticed before I did. Morpheus8 alone produces a known result. Morpheus8 paired with YOU was different. The recovery profile was visibly faster, and the skin quality at one week looked like what I would have expected at three weeks.
I share that observation with patients before I share the table. They appreciate it.
