BBL Recovery Week by Week: What Actually Happens After a Brazilian Butt Lift

I just saw a patient last month who flew in from Dallas specifically for a Brazilian butt lift consultation, and she arrived with a printed list of questions. Not one of the questions was about the surgery. They were all about recovery: when she can sit, when she can fly back home, and when she can get back into working out at the gym.

Smart list. The operation is my job. The recovery is where the patient makes half the result, maybe more. So the map I gave her is the one I give every patient.

The First Week: Sore, Not Sick

Post-surgery is the most painful period, but even then, many patients find it less extreme than they had assumed. A BBL is a comparatively less intrusive surgery I perform. The pain will feel like your first day back at the gym; like you have been worked over and are sore with any slight movement, but you can manage to push through it and move about relatively normally. This discomfort lingers for about five to ten days post-operation.

You will have a drain for a 5 to 7-day period in your liposuction tummy area. You will wear a pressure compression garment over all your liposuctioned areas, which will decrease swelling and shape the contour created in surgery, both equally important. Studies have led us to pick for our patients the highest quality pressure-set garments, as opposed to having patients research and guess online.

Most get back to work five to seven days later, but on light duty. NO lifting. Still no gym yet.

The Sitting Rules, and How Strict They Really Are

This is my single, hard and fast rule: no sitting on hard surfaces for two weeks after surgery. Pressure is death to fat grafts.

The workaround is the BBL pillow, aka the booty pillow. You sit resting on your thighs, and the pillow keeps the buttock floating free. That removes pressure from the graft entirely. For sleeping, stomach position is the goal. If another procedure makes that impossible for you, we provide wedges that hold you on your back in a beach chair position with no weight on the fat. You only have to maintain all of it, pillow and sleeping both, for about two weeks total, not for months.

Almost all of my patients having this procedure need day five clearance, which means their sutures come out, drain tubes generally get removed, and they are good to travel back to their homes out of state without any added risk.

How Much of the Fat Survives

People ask the question constantly. The number is fifty to eighty percent, and which end of that range you land on is never just a matter of chance. It comes down to how well the transferred fat reconnects to a blood supply in its new home, and there are two levers that move it.

First, the patient must follow instructions about sitting on their buttocks during the initial two weeks. Second, in the OR, I wash all harvested fat with AuraClens, which is a solution containing Poloxamer 188. This wash eliminates nonviable material, fluid, dead fat and oils that would otherwise take up space in the syringe to ensure only fat capable of staying in the recipient site is injected. Besides removing waste, Poloxamer 188 repairs the adipocyte cell membrane and stabilizes the cells by sealing damaged membranes and guarding the cells against apoptosis, programmed cell death. As a result, more fat cells survive and stay in the injected location.

So you will see that final result at around week 4 and it will stabilize there with the definitive result somewhere around 3-6 months.

The Massages Are Not Optional

And it might be surprising that if I had patients become obsessed with any one specific recovery item, it might be this, lymphatic massage, performed two or three times weekly in weeks one through four, with as many times as you could cram into your first month.

The lymphatic drainage massages that we administer break down any fibrosis formed in the treated areas from the liposuction, drain off fluid that needs to exit the body and transition liposuctioned zones from potentially bumpy textures to smooth contours. Our patients book their post-surgical massage therapy sessions with the program provided by Southwest Plastic Surgery. This means that the therapists who work with them do this all day, every single day.

What Changes When You Combine Procedures

Breast augmentation / rhinoplasty will not change anything. But tummy tuck makes the process take ~ 2 weeks to normal activities, 6 weeks to exercise.

Supercharged BBL, also known as BBL with implants (a combination surgery utilizing autologous fat transfer and buttock implants), uses a different timeline. Because the buttock implants sit inside the gluteal muscle, there is more pain initially compared to fat transfer alone. Typically, two weeks of activity at home are manageable, with around four to six weeks until the return to normal exercise routines.

Protecting the Result for Years, Not Weeks

After the first month, the whole game is weight stability. When you lose weight, the transferred fat mass also shrinks. When you gain weight, the fat volume expands, stretching your skin. The worst pattern is fluctuating, as it repeatedly stretches the skin, contributes to sagging and results in the irregular topography that patients later want addressed.

Possible complications with liposuction are rare. As in all surgeries, there is a small risk of infection (defined by redness, pus drainage, worsening focused pain). Additionally, seromas (fluid pockets) can occur in the suctioned regions. Please contact us if you experience these or any other worrisome symptoms.
